Assuming that what you are talking about is what Word calls an "online
form," follow the links at

Chapman's series of articles. You may also want to look at
http://www.word.mvps.org/FAQs/TblsFldsFms/LinesInForms.htm. Chapman's series
are the best tutorials I know of and take you from basic forms to userforms.
