To clarify one point: The original post was about a custom image for a button on
the Quick Access Toolbar, and that is still not possible. What Greg's page
discusses is adding a custom button to a custom group on the ribbon itself.
Once you get past the apparent complexity of RibbonML and realize that it's just
some copy/paste, this may actually be preferable.
