Word isn't really constructed for this, but...
You could put the static text (the $) and the space to type in in two table
cells. Then add a bottom border to the table cell where people should type.
Possibly, what you want to do is create an online form? You'll find a
tutorial on how to do this at www.mousetrax.com, and more information at
word.mvps.org and then on my website.
