Auto-Entry for a document

I am trying to format my document so that when i open it, it will prompt me
to "fill in the blanks" within the document itself. I am trying to be able to
not have to erase old information and add in new information each time i open
it. It is a Delivery ticket for my sheetmetal department and has several
places in the document where information must be manually entered into the
blanks. (i.e. the prices, who received the ticket, what work was done, the
job #, the date, etc...) I thought there was a way to do this automatically
on Mic. Word but i can't figure it out. any help would be GREATLY appreciated!
