I have a template that I have form check boxes in. This template needs to be
emailed. When it's sent via email, the check boxes disappear. The text
fields are fine.

Send the document as an attachment, not as the body of an email message. Word
must convert its document file format to something email programs can work with
(plain text, HTML, etc). The converters don't convert checkboxes.
