I didn't knowingly make any changes

I was referring to exactly which actions you took when you opened Normal.dot.
It's still not completely clear to me what you did, there. Do you actually have
text in the header/footer of your Normal.dot? Do you see bold and italic in the
Normal.dot?

Please do check the style settings. It's remotely possible that some text in
some document was formatted unknowingly with these styles, that it was selected
and formatted bold/italic and that the style was set to automatically update
back to the template.

However...

The issue was that the very next blank document came up just as before
changing normal.dot. That is what I didn't/don't get. It's like something
else is over-riding the settings in normal.dot. That just doesn't make sense
to me.

Besides what I described above, it could be also be an Add-in.

I suggest you try:

1. Rename Normal.dot to Normal.OLD
2. Restart Word. Do you see the problem in a new document?
3. If not, then the problem was in Normal.dot
4. If you do still see the problem, quit Word.
5. Delete the new Normal.dot that Word just generated (you still have
Normal.OLD)
6. Hold down Ctrl and start Word again, to force it into "Safe mode". Is the
problem gone now? If yes, then you have an Add-in of some kind that's doing
this.
