most likely when it's not really "text" anymore (a raster or vector image). Ordinary text should not become a raster image, but if one applies, say, special effects to the text (think WordArt etc.), this will happen. The vector part is less likely, because it would be all or nothing: all text will be saved as outlines, or not.
