Was the document opened in Reading Layout view? That's a common complaint.
Updating the TOC (F9) should fix it, and you can disable Reading Layout view
on the General tab of Tools | Options?

I have a document numbered consecutively, I right click on the TOC toupdate
it, page numbers show correctly, save the document, close it and reopen itwhole
and all the page numbers in the TOC show up as page 3. Went through the
process again with the same results.
