Caption numbering fails to correctly reference heading text number

When I insert captions (figure, table, etc.) and choose to include the
chapter number referenced to heading text, the correct caption will display
in the caption pop up box, but when I click "Ok" to insert the caption, the
field reverts to 0. The inserted field is always {STYLEREF # \s} where # is
the heading level to be referred to. I noticed that section breaks effect the
field reference, but in this particular case, I only have a small table
between the chapter heading and the table I am trying to caption.
