The disappearance of those commands often means either the presence of a virus, a damaged normal.dot file, or damaged registry entries. Assuming it's not a virus, try the steps shown here to diagnose and isolate the problem:

Office 2003, SP2, SP Professional SP2, Server 2003 network. Several users,
individually over the last few months have had their Word Save and Save As
commands dimmed. No other Office aps or commands affected. No other user
profile, if present, is affected. Problem can be fixed by creating a new user
profile in Windows. Is there a way to fix this without having to start the
user with a fresh profile and thus losing many of the application and
personal settings?
