Within-document links break when migrating from Word 2000 to Word 2003

I have a document created with Word 2000 that has a large number of
links each of which goes from one part of the document to another part
of the same document.

The links were created by highlighting what I wanted to link and
copying it to the clipboard, then going to where I wanted to insert
the linked copy and hitting Edit->Paste Special->Paste Link->Formatted
Text (RTF).

I can open that document in different Word 2000 installations, and
even OpenOffice.org Writer, and the links all work correctly. Open
the document in Word 2003 and the links are all broken with error
messages like "Error: Invalid Link".

How can I fix this without having to strip out all the links and do
them over in Word 2003 which at this point isn't really an option?
