Installable ISR - still having problems

Has anybody out there created there own Installable ISR not using the GIISR.DLL.
I'm having problems and would like to see how they got it working.
My installable ISR loads and then it looks like I'm getting an Exception 00e (whatever that means)
in device.exe.

Please help me.
