This bit sounds exactly like the issue I'm having on a W2K server SP4.
Little icon appearing, click it, it disappears, only to come back
later on.
At first I suspected a BITS issue, since I couldn't start that service
for whatever reason. Using the BITSADMIN.EXE utility from Windows XP
Support Tools it fixed that tidbit... Now I'm just plain faced with
the error depicted in the subject of this thread.
For me aswell, all the solutions listed on

http://update.microsoft.com/windowsupdate/v6/showarticle.aspx?articleid=20&ln=en
did not solve the issue. At first it was a tad annoying, but by now
it's grown into something that annoys me to no end...
The account used has worked normally without a hitch and it is a
domain account. As I'm the admin of this system, I did try anything to
adding the account to any group that might add rights to the account
that it was lacking. The only group it isn't a member of is the
'Guest' group, and there is no such thing as a 'limited' group under
W2K.
Oh, and for those wondering: .NET isn't installed. I don't need it on
the machine. So deinstalling it is kinda difficult to do ;-P
All applicable services (Automatic Updates, BITS, RPC) are all set to
use the Local System Account. So I doubt that is the cause of the
issue.
If anyone does have idea's I'm curious to hear 'm.
