Most commonly caused by third party firewalls/AV/ad blockers etc. such as
Zone Alarm (here's a list of some programs which are known to cause problems

Also, make sure
a.. Active Scripting is turned on.
a.. Internet Explorer is configured to permit the download and
initialization of ActiveX Controls.
Click Tools, Internet Options, Security tab.
Click Internet Zone, Custom Level.
The following settings should be set to Enable or Prompt:
a.. Download signed ActiveX controls
b.. Run ActiveX controls and plug-ins
c.. Script ActiveX controls marked safe for scripting
d.. Active Scripting
