Hi Blake,
Can you please confirm that the license server is in TSLS group?
Also, please check the 'Review Configuration' on license server and verify that every thing is shown okay there also? (For opening 'Review Configuration', click on the server name in TS license manager and right click. You will get an option for 'Review Configuration' there)
Is there any way that you can create a 2 way trust?

Well this morning my worst fears were realized. Even though the license diagnostic tool says everything is A-OK, terminal server is no longer getting valid licenses. The grace period has ended and now nothing works. The one-way trust works for me to log into the TS server via RDP, but I am immediately booted out as (apparently) a license is not acquired.

AFAIK, Per User licensing is not enforced in 2008, but I have not tested this scenario.

The Enterprise Admin for the 2003 AD should have run adprep
before allowing a 2008 member server. Can you verify this?
I expect that you will find EventId 4105 on your TS LS. Part of
this event's description is:
"The Terminal Services license server cannot update the license
attributes for user "xxx" in the Active Directory Domain "yyy".
Ensure that the computer account for the license server is a
member of Terminal Server License Servers group in Active
Directory domain "yyy".
And that's what needs to be done.

The domain where the users reside (and where the TS license
server is a member) is Windows 2003 AD. I have no control
over it, sadly. (If I did I'd setup the 2 way trust)

Blake, you don't have to worry about refused connections,
because in W2008, Per User licenses are issued and trackable,
but AFAIK they are not enforced.
Moreover, your grace period has already ended, by definition
it ends as soon as the TS discovers a LS (or after 120 days).
But you still need to solve the problem.
Did you upgrade the AD schema before joining the 2008 servers
to the domain (with adprep and forestprep)?
Have you checked the EventLog on the TS and the LS? Maybe
you're seeing event 4105 on the LS, stating that the LS
cannot update the user account properties?

Hotfix applied, behavior unchanged
When I run the licensing diagnosis tool, it says everything
is fine. But when it says there are no CALs in use, it
worries me.
Does this matter - the TS servers is 2008, the TS Licensing
server is 2008, but the AD is 2003. Does that matter?
I just don't want my grace period for TS to run out and
everything screech to a halt when I have the CALs legally
purchased.

After much consternation
I have a 2008 TS server and a 2008 TS License server
(two separate boxes)
I log into the TS server as a domain user (who is
also a local admin) and run the licensing diagnosis
toll - I get an 'A OK' message. It displays my 50
PerUser CALs from the license server.
I try to run a report on the TS License server. It
shows me 50 CALs available, none in use.
I *am* within the grace period after the initial
install, as my TS server has been up for a couple
weeks and I just setup the license server today.
How can I guarantee my license stuff is working
properly??
