They've named the box or the internal domain to be the same as the external
domain?

If the internal domain has the same name as the external, as far as I'm
concerned that's the best way to do it. Especially if you are looking at
deploying Outlook/Exchange using RPC over HTTP. It makes name resolution
work inside and outside your network.

No need. Just create an A record for www on the internal DNS that points to
the external website.

As far as I know Windows 2000 doesn't allow you to rename the domain, so it
would have to be after the upgrade, but I wouldn't bother. Just create the
DNS record and you will be fine.
