regarding SBS CALs, as I understand it, that if you buy Premium, the 5 built in CALs would be premium CALs, so yes you would only need 5 more premium CALs

SBS Premium will include the Second Windows Server Standard license so that's covered. And yes, if all 10 users are going to access TS, you'll need 10 TS cals

And to be totally clear, you can install SQL on either the SBS server or the second server included in Premium.

So just to clarify:
