Thanks for posting here and also thanks for Chad's input.
As I understand it, the issue is:
You have installed WSUS 3.0 on the SBS 2003 Prem server which has ISA 2004
server installed. Now the "WSUS is reporting the client IP address as
itself" so that you would like to find a way to bypass ISA entirely on WSUS
communications or add a rule which will "expose the correct IP address".
If I have misunderstood your concerns please feel free to let me know.
Before we go further, I would like to clarify that:
1. What do you mean by "WSUS is reporting the client IP address as itself"?
Does the client has incorrect IP displayed in the "All Computers" list on
WSUS 3.0 console?
2. Do you mean WSUS client registered incorrect IP address in WSUS admin
console?
3. As Chad asked, are you seeing this behavior with internal (LAN) clients,
external (WAN) clients, or both?
Based on research, if WSUS client registered incorrect IP address in WSUS
admin console, we may consider the following steps to troubleshoot this
issue:
Reset the proxy list (and bypass list) on problematic client computers

1. Open Internet Explorer.
2. On the Tools menu, click Internet Options.
3. Click the Connections tab, click LAN Settings, and then click Advance.
4. Delete any entry in the Exceptions section.
5. Quit Internet Explorer.
6. Click Start, click Run, type cmd, and then click OK.
7. Type the following commands. Press ENTER after each command.
Proxycfg -u
proxycfg -d
net stop wuauserv
net start wuauserv
Wuauclt /detectnow
Wuauclt /reportnow
Now please refresh the WSUS console and check the issue again.

Meanwhile, the following article describes how WinSock Proxy Auto Detect
(WSPAD) is used to detect Internet Security and Acceleration (ISA) Server
computers on the network

Regarding the ISA server "All Protected Networks", ISA Server protected
networks are included in the All Protected Networks network set configured
by default after ISA Server installation. This network set contains all ISA
Server networks except the External network. After installation, there are
two predefined network sets: All Networks and All Protected Networks
(containing all ISA Server networks except the External network). When you
create a new network, this network is automatically added to the All
Networks network set.
Therefore, this rule is not the problem to block WSUS communication, on the
contrary, it helps to ensure all internal traffic are getting through
including WSUS communication between clients are SBS server.
In order to get the issue resolved more efficiently, please help me collect
some information for further investigation:

1. Please capture some screenshot when the error/problematic messages

2. Please gather an MPS setup report:
1) Please download the MPSRPT_SETUPPerf.EXE from the following link and
then run this tool to gather some information from the problematic computer:

2) Double-click on the MPSRPT_SETUPPerf.EXE file.
[Note] This process may take some time; however, it will not have a
negative effect on the performance.
3) A CAB file will be generated in the
%systemroot%\MPSReports\Setup\Reports\Cab directory called
%COMPUTERNAME%_MPSReports.CAB. The CAB file will contain the reports
generated by the MPS Reporting Tool.
Send the .cab file directly to me at v-robmen@xxxxxxxxxxxxx .
