Susan,
Yes, a call to PSS is always an option and perhaps my next one. In all, it has cost me three multi-hour sessions on weekends spread out over the past 11 months. Not a huge amount of time but definately enough to have run the gauntlett of published diagnostic procedures. I have been (for the most part) just using OWA and a VPN connection for 'normal' Outlook access. I recently got sick of this state of affairs (again) and this time I am going to solve it!
With respect to Vista I followed the procedure in http://support.microsoft.com/kb/926505 which is to say I verifed that SBS2003 SP1 was installed, applied the update for Vista/OL2K7 compatibility, installed the WMI update, etc. As it was a Vista-x64 client the actuall joining of the client to the domain went a little rough but that is not the problem. I am troublshooting the HTTP/RPC issue from my trusty XP SP3 system that was joined to the SBS domain years ago using the very straight forward process of adding a workstation to the domain via the wizard and web install process.
And yes, I have run the CEICW (many times) and yes I have followed the instructions in the RWW for how to configure the Outlook profile. That is how I originally set my OL profile years ago and it worked without change until the server changes were applied. I have of course re-verified the OL profile settings on both Outlook2003 and Outlook2007.
In the mix somewhere Exchange 2K3 SP1 and SP2 both got applied as well and WS2K3 SP2 got applied.
Thanks for thinking about my issue.

In all of this self help and beating your head against a wall how much as this cost you? Honestly when I get to this level of frustration, if I were in your shoes I'd get the credit card out and call MS support.
When you said you made SBS work with Vista what did you do?
You've rerun the CEICW and you've gone into connectcomputer and you've followed the Outlook over http instructions on the workstation?
Next run www.sbsbpa.com
Nothing in those updates should have impacted this though.
