One more thing you could try. Go into Services and temporarily stop the SQL instances, and see if you get a better result with Outlook then. That might indicate a problem with resources on the SBS. There are ways to throttle back the memory usage of the SQL instances, but there's no point in considering that until you're sure it'll make a difference.
I would expect that if your Exchange databases are cramped for space, that could be an issue. You're doing regular online backups every night to clear out the Exchange transaction logs, right? I would expect that Exchange would complain like crazy if it had insufficient space (in the application log), and if it runs out, it'll dismount the databases. I keep at least 20% free space on my Exchange partition, and that's 20% of the maximum - including all the logs it's created up to right before the online backup.
Antivirus configuration could play a part in this, although that seems doubtful given that the problem occurred right after installing ISA. If I had to take one wild guess, I'd think maybe you've got ISA logging to SQL, and the SQL logging database is grabbing up RAM. But I'd start by testing with my first suggestion, shutting down the SQL instances to see if that helps the Outlook/Exchange issue.

Yes, I did run CIECW after the install, as per instructed. I assumed that would configure the rules as normal.
I've been checking the logs on the server very closely since ISA was installed, and nothing I consider as significant has appeared. It hadn't occurred to me to check the event logs on the workstations. The only thing I can find in the Application event log regarding Outlook is this
"The store C:\Users\<deleted>\AppData\Local\Microsoft\Outlook\SharePoint Lists.pst has detected a catalog checkpoint." There is nothing in the System log.
I have had trouble with memory issues on the server. The server has access to 4GB of RAM, but usage displays that about 3.5GB of that is regularly in use. The box has 2 Quad Pro processors, and frankly they are hardly being touched by the usage on the server. There is a lot running on that server with various instances of SQL Server, (a notorious memory-gobbler). Hard Disk space is running short on the system drive, with about 3.7GB free on a 40GB partition. I had wondered whether that might be slowing Exchange 2003 down.
Otherwise everything seems to functioning as normal. Only other thing I had noticed was a slow down in OWA.
I will try your suggestion about creating a new mail profile and see if that has any effect.

Regardless, it doesn't seem like installing ISA on the server should effect Outlook performance. Have you checked the logs on the SBS and the workstations to see if any errors are being logged? Have you looked at Task Manager to see if there's any kind of an issue with CPU or RAM?
Are you having any other symptoms from these workstations? For example, does access to file shares and the Internet appear to be functioning normally?
What happens if you create a new mail profile for one user - still slow?
