Please allow me to confirm that my understandings are correct. As I
understand it, the issue is:

You company name is displayed twice at the top of the fax.

If I have misunderstood your concerns please feel free to let me know.

Suggestion :
==============
For your issue, I would like to suggest that you perform following tests
to see how it goes:

a. On the SBS Server Console, go to Start -> All Programs -> Accessories
-> Communications -> Fax -> Send a Fax -> Run the Send Fax Wizard, will you
encounter this issue?
b. When you running the Send Fax Wizard, proceed to the "preparing the
Cover Page", click "Sender Information", make sure you have input your
Company name only once.

If we cannot resolve the issue after we perform the above steps, please
help me collect some information for further investigation:

Information Need
==============
1. Check event viewer, please help save Application part to *.evt and send
to me.
2. When did the issue occur? Did you experience this issue before? If no,
did you make any changes or install any updates/software on the SBS Server
recently?
3. Does this issue occur to every outgoing faxes or only certain?
4. How do you know the Company name shown twice? Please capture a screen
shot for it and send it to my mailbox (v-gzwang@xxxxxxxxxxxxx).
5. Does this issue occur to all clients or only certain?
6. Does this issue occur on the Fax Cover Page, Fax Body content or both?
7. Please help to gather following information:

a. Event log entries from Fax.
b. Activity Log file

%ALLUSERPROFILE%\Application Data\Microsoft\Windows
NT\MSFax\ActivityLog\InboxLOG.txt.
%ALLUSERPROFILE%\Application Data\Microsoft\Windows
NT\MSFax\ActivityLog\OutboxLOG.txt.

c. PSS log

%windir%\ system32\LogFiles\Fax\Incoming\FSPRxxx.log.
%windir%\ system32\LogFiles\Fax\Outgoing\FSPSxxx.log.

Please pick the latest file, which should be the one that represents the
most recently failed operation.
