SBS 2003 Premium or Standard?
2 NICs in the server or 1?
Have you tried: disable RRAS, re-run CEICW, enable the firewall, select the
services and then complete the rest of CEICW? Then reboot to test.
You may also want to download, install, update and run the SBS 2003 BPA to
see if it can find any problems:

You could always write a batch file that runs at startup to kill ipnat.sys,
but I think you really need to find the source of the problem.

I have a SBS2003 R2server. Several months ago after some updates, clients

longer had access to the sever, dhcp did not work, etc. After 6 hours on a
support call with MS, we got it working. MS was never clear as to the
problem. Next updates, same problem. This time, had a much more
knowledgeable
person from MS. Figured out the problem was IPNAT in an hour. Ran command
"net stop ipnat" and everything went back working. Unfortunately, the
client
had 30-40 employees waiting for paychecks, I was unable to work with MS on
the cause of the problem. It appears now anytime the server is restarted,
I
have to run the stop command. How can I prevent ipnat from running on
startup.
