Sounds like a DNS issue, not ISA. SBS runs the DNS server service and
forwards all external requests to the DNS you provide in CEICW. Try using
nslookup on the console to various sites and verify the DNS used in the SBS
server (it should be).
Verify that the DNS addresses setup in CEICW haven't been changed by your
ISP. Some ISP's don't inform you of changes to their DNS server addresses
so you should check them periodically. Also don't use microsoft.com for
testing as MS has some default hooks in various programs to make sure it
works.

I've had this issue for a few weeks now. Running SBS 2003 SP2 R2 with ISA
Server 2004. The update to SP2 & R2 were done a long time ago. This
problem only started recently.
If I try to browse to any of the microsoft.com sites from a workstation,
it almost always times out. Occasionally I get a very slow connection, but
not often. If I connect a workstation outside of the SBS 2003 server,
directly to the DSL router, it connects instantly and works just like
you'd expect. All other web sites are fine behind the SBS. If I browse to
the IP address corresponding to www.microsoft.com (207.46.19.190) the
connection is fine from behind SBS.
OK, of course now that I write this post it's working! I just logged onto
the server to see if I could browse to microsoft.com from there. It worked
fine. Then I retested from my workstation, and it worked fine once, and
then stopped again. For the last 4 hours, and most of the last 4 days, my
workstation could not connect at all.
I tried disabling all the ISA firewall rules that referred to microsoft
websites, both in Firewall Policy and System Policy. Nothing made a
difference. I tried enabling ISA web caching and disabling. No difference.
Any suggestions? I can't think of what else to try.
