Exchange server was previously configured as you'd suggested (i checked to
be sure). Please review response to Claus.
This server is primarily used for e-mail for a small organization (though
they are heavy e-mail users) and as a print server; not much else runs
on/through this box.
This am, things working fine (smtp sever up, two messages in queue and one
incoming connection); Internet speed at expected levels and consistent.
I'm about at wits end (short drive on a down hill street most would say.....).

suggestion - that your SMTP service is sending out mass emails. However, it
doesn't mean that you've been hacked (although you may have been), you may
find that your server is configured to allow it to be used for RNDR spam
attacks (see http://www.cryer.co.uk/glossary/r/reverse_ndr.htm). If you open
Exchange System Manger, navigate down to your server and then look at the
"Queues", if you have lots of outbound emails awaiting delivery and the
majority of those are non-delivery notifications then you can be sure that a
spammer is making use of your server. If this is the case then the above
link contains links to pages with notes on what to do to fix it.
If that isn't the problem then the rest of Claus' suggestions look good,
can't think of anything to add.

Have you looked at the traffic that goes out to the Internet when you
start the service? Did you get hacked and your SMTP is sending out mass
email? Have you looked at the memory and cpu usage of the service?
Is the SBS box a 1NIC or 2NIC setup? If it is a 2NIC setup, disconnect the
LAN NIC from the switch and see if you still have the issue. If not, this
is most likely coming from one of your workstations.
When you rebooted the server before, how long did it take for the symptom
to show up again?

Situation: wherever the smtp virutal server is running; internet
connection
speed slows to a crawl.
Running Windows Small Business Server 2003 w/ sp2. Internet service is
available via a seperate server running ISA. If smtp virutal server is
enabled Internet speed slows to crawl; is stopped Internet speed returns
to
normal. Nothing that would account for this beheavior appears in event
logs
and while I've seen evidence of this beheavior before, generally a reboot
of
the serrver resolves the issue, until today when nothing (we're tried so
far)
seems to resolve the issue.
Any help, suggestions would be appreciated.
