Sorry if someone may have mentioned this already, but is there a postini type spam service in use? I've seen this happen when the user in question doesn't have an account on the postini/spam gateway. This could explain why no NDR is being delivered... these types of services spool messages for up to 7 days.

Actually it wasn't a RUS fault, rather an issue with the user
attributes that caused RUS to not generate SMTP addresses for those
users.

Do the users have the proper external smtp addresses or not?

Check the users properties specifically their email addresses.
I've had some users in the past that Recpient Update Service has
issues with generating an external email address. Often these
users were not created correctly.
