You can install SQL Server WE without having to install ISA. There is no requirement to have both on it. The word "upgrade", in your case since you have SBS 2003 SP1, is used for licensing purposes. If it will make you feel better give Microsoft Licensing a call

We need the SQL Server 2005 WE and won't be needing the ISA 2004. I assume it is possible to have one without the other?
So is it possible to upgrade from SBS 2003 Standard SP1 to R2 Primium to take advantage of the SQL Server 2005 WE?
I'm either blind but as all I see is upgrade path from SBS 2003 Standard SP1 to SBS 2003 Standard R2.

R2 is a set of technologies on top of SBS 2003 SP1. As far as going from Standard to the Premium version has to do with whether you want to install ISA 2004 and SQL Server. All in all, you are adding components to your server.
When you install R2, you can upgrade the WSUS 2.0 component to the 3.0 version or skip the 2.0 installation completely and just download and install 3.0 directly. If everything goes well you'll just have to restart the server and you should be good to go. If you decide to install ISA, you have to plan in advance which way your clients will be connecting to the internet. If you decide to install the Firewall Client it will take some time to deploy it to the workstations. You'll also have to re-run CEICW to configure the rules. For the R2 part you don't need a full working day of downtime to do that. The ISA roll out might take more time but but it shouldn't be a network shutdown since you can work on each workstation individually

Would it be a total reinstall or an upgrade over existing version? Being only our server, I am concern with Exchange mailboxes, AD objects, MSDE instances for security and backup software not properly replicated and downtime if we wanted to carry out this upgrade on a normal working day.
what would be a realistic timescale? We are small with one server and 15 users.
