Time to break out the AD tools (DCDiag, Netdiag) and see what's up.

dcdiag /c / v
and
netdiag

from both servers and post if needed.

OK, a bit more to this...

The two AD still appear to be synching, if I create a test user in one
it does show up in the other and vice versa.

AD Sites and Services SBS > resynch with Win2K fails - tombstone
Win2K > resynch with SBS2003 seems to work OK.

AD Sites and Services Win2K > resynch with Win2K works OK
Win2K > resynch with SBS2003 fails - tombstone

If I try and create a user on the SBS2003 which includes and e-mail
address then it fails to create the user.

I've got event ID 8231 MSExchangeAL appearing over and over in Event
log.
