no...you can do either one first...or at the same time
Understand that it could take 24-48 before all DNS servers worldwide are
updated with the new MX records
In the mean time...keep the old MX record to...just so mail doesn't get lost
Just me the one coming to SBS a higher priority (lower number) than the one
to the ISP

According to your description, I understand that you want to publish your
OWA. If I have misunderstood the problem, please don't hesitate to let me
know.
Larry and Russ are correct, we have to confirm that OWA can work internal
first, you can try to access the OWA on SBS via
https://localhost/exchang/
to test. Then we have to run the CEICW to publish the OWA web site and
create CA for the OWA access:
Go through the follow KB and Rerun CEICW again carefully.

Detailed steps for your reference:
a. On the SBS 2003 Server open the Server Management console. Go to
Standard Management\To Do List.
b. Click the "Connect to the Internet" link.
c. When navigating to the Firewall page, select "Enable firewall" and
click
Next (I suppose you have 2 network adapters in SBS 2003).
d. On the "Services Configuration" page, select all the items and then
click Next.
e. On the "Web Services Configuration" page, make sure "Allow access to
the
entire Web site from the Internet" is selected. If you select "Allow
access
to only the following Web site services from the Internet", make sure all
item in the list are selected. Click Next.
Note: This step will publish OWA
f. On the "Web Server Certificate" page, choose to create a new Web
server
certificate and then type the public FQDN (your public DNS name) that you
will use to access OWA and RWW (for example, if your public FQDN that you
use to access the sites is www.xyz.com, you should type www.xyz.com as
the
new certificate name, if you do not have public FQDN, you also can input
your public IP address here).
Note: This step will create self-sign Certificate to help you access OWA
via https from external.
g. Go through the remaining steps.
And last, you have to ensure your router before SBS has forward port TCP
443 to SBS. You can telnet port 443 from external to test.
