Sorry that didn't help, I'm running SBS 2003 here as well, but not ISA
and that was the issue I ran into.

It has to do with an outgoing pptp vpm connection being blocked, which
you say the rule exists for, I wonder if the note at the end regarding
the rule placement may cause a issue, the cause listed seems to make
some sense when your stating that the connection is denied, and you are
getting a winsock error:
The ISA Server policies that are created by the SBS connection wizard
require user authentication. The ISA Firewall Client provides user
credentials for non-HTTP traffic. To create the PPTP connection, the
PPTP client must use TCP (IP protocol 17) on port 1723 and the GRE (IP
protocol 47) protocol. The ISA Server Firewall Client only processes
TCP and UDP traffic that is handled by Winsock. Because the ISA Server
Firewall Client cannot process the GRE traffic, it cannot authenticate
this traffic to ISA Server. Therefore, the GRE connections are denied,
and the PPTP connection attempts are blocked.
The Cox modem, is it a true standard adsl modem, or is a Speedtouch?
I've heard of problems with speedtouch modems blocking GRE, since they
aren't a true modem, but a NAT router as well.
Thats about all I have, Sorry I couldn't help. The only thing I tend to
do when sbs starts to act up, its usually because I've made a change to
IIS, RAS, etc.. and not used the appropriate wizard instead. Usually
re-running the CEICW wizard tends to repair connection issues, internal
or external quite well.
