Copying mail to external mail recipient

SBS 2003 all patched up to date. Mail using SMTP connector through ISP
smarthost. Mail collection from SMTP too.
I have one Exchange mailbox user that would like all mail copied on to an
external e-mail address (ie delivered to Exchange mailbox and his Hotmail
account).
If e-mail is sent internally (ie from another Exchange mail user) to the
internal (Exchange) e-mail address, then it is successfully delivered to
both addresses, internal and the alternate external recipient.
If mail comes in from an external source (using SMTP mail feed) then the
mail is delivered to the Exchange mailbox, but not copied on to the external
alternate hotmail address.
If SMTP through ISP smarthost is used for delivery, then Message tracking
shows:-
SERVERNAME.DomainName.co.uk #5.5.0 smtp;554 external@xxxxxxxxxxxxxxxxxxxxx:
Sender address rejected: Access denied
If DNS is used for delivery, then the spam blocking rules on the sending
server reject the message because the IP address being used is in a blocked
range (residential range).
Any ideas?
