I've no idea why WSUS or Windows Update didn't show me the 916106 patch but
manually installing that has cured this problem. Many thanks for your help :)

What ISA patches do you have specifically?

You need ISA sp2 and http://support.microsoft.com/kb/916106/en-us..
it's the 916106 that is the key one.

Go to Microsoft update or install WSUS and get the ISA sp2 and the post
SP2 fix up

And then go see what other patches you are missing.

If you were updating this server you would get these patches already and
this wouldn't be an issue.
