People say 'fully patched' and don't necessarily mean that though (sorry for the tude though0
What ISA patches do you have specifically?
You need ISA sp2 and http://support.microsoft.com/kb/916106/en-us.. it's the 916106 that is the key one.

And then go see what other patches you are missing.
If you were updating this server you would get these patches already and this wouldn't be an issue.

ISA shows that it is allowing the app through but when accessing the Download Store, iTunes errors saying that the network connection is inactive (error 502).
