ALL my SBS's are dual NIC with simple NAT router. Until recently (when the
ADSL modem started playing up and was replaced with a nonNAT capable device)
my LoungeAN had NAT at the modem, NAT at a router used to isolate my systems
from my housemate's, and ISA (2 NICs, of course).
I also promote this WAP outside ISA for domain members using wireless at the
office. They attach to the less secure WAP network then VPN into the
'office' network. The advantage here is that all WAP access is 'external' to
the network.
Do I care that one of my visitors may visit a client on another floor and
borrow my internet connection? For the most part no, but this is why I use
both wireless security (WEP/WPA-PSK/whatever) and MAC filtering, when they
leave I can, theoretically, disable their MAC address. I'm more likely to
say to them 'We'll leave your info in the device, should you visit again
you'll have access, if you're nearby and need internet just let me know.'.

Does it cause any 'issues' if the SBS box is set up with *DUAL* NICs eg,
is does DUAL NAT'ing then occur?
And does that then mean that wireless visitors then know the WPA key and
could, in theory, come back and re-establish an internet (if not intranet)
connection with the key - or is there a way to handle this issue?

The easiest way is to place a router between SBS and the internet, I
often suggest a four port+Wireless. 1 port for the server, leaves three
physical ports, 1 goes to the boardroom to a wallplate marked 'Internet'
and the others can be patched to a particular area as necessary. Visitors
with wireless need to advise of their MAC address and are supplied the
'key' upon request.
