By default, the out-of-office reply to Internet recipients is turned off.
Many administrators do not allow out-of-office replies to be sent outside

5. Stop the Simple Mail Transfer Protocol (SMTP) and Microsoft Exchange
Routing Engine services, and then restart them.

After above steps, please test your issue again. If the problem persists,
please let me know the following information.

1. Please le me know if the users inside SBS org could receive the OOF?

2. Did you see any error event related with OOF in the event log?

3. If you create a new user, could this issue be reproduced?

4. If you create a new Outlook profile, does the issue persist?

5. Check the rules in the Outlook. Disable all rules and have a test.


Moreover, OOF assistant sends OOF replies to every sender only once. i.e.
if you receive an OOF reply from this user once, when you send another
message to the user, you will not receive the OOF reply again until the
user has disabled and re-enabled OOF assistant. Thus, every time before you
tested this issue, please disable and re-enable OOF assistant.
