I'm not sure this is on the client side now. I had another user send to this
recipient and the same thing happened. Yesterday when I tried it, I thought
it wasn't an issue, but I hadn't refreshed the queue but the test messages
sent to the same recipient produces the same results.
It appears it's happening no matter which computer/Outlook profile we use to
send the message.
They are using SBS 2003 with the mail profiles on the Exchange server. They
are sending via DNS and picking up POP mail with the POP3 connector.

From your post, my understanding on this issue is: when the user send
message to a recipient, the recipient receive multiple copies of the same
message. If I am off base, please feel free to let me know.
Based on my knowledge, I suggest you perform the following steps to narrow
down the issue:
1. Disable all rules
==================
Please go to Tools->Rules and Alerts, and uncheck all rules to test if
this
issue still occurs.
2. Create a new PST file if you are using PST files for default delivery.
To check the delivery setting, refer to the following steps:
a. Open Control Panel.
b. Open Mail.
c. Click E-mail Accounts.
d. Choose "View or change existing e-mail accounts" and click Next.
e. You can check the Delivery e-mail location now.
3. New Outlook profile
=============
Please create a new profile to isolate the issue.
For the steps on how to create a new profile, refer to the following

4. Outlook Safe Mode
=================
a. Click Start, and then point to Programs.
b. Press CTRL while you click the shortcut for Outlook that you want to
start.
c. Hold down CTRL until you receive a message similar to the following:
"Outlook has detected that you are holding down the CTRL key. Do you want
to start Outlook in safe mode?" Click Yes.
5. Disable third-part applications
==================
Please disable all the third-party software, especially the antivirus
applications.
6. Test with another POP3 account
===================
Please configure a new profile using another POP3 account with "Leave a
copy of messages on the server" option selected, and check the results.
