Are you using Trend Micro CSM suite?
This is a common issue that can be fixed with a patch from Trend

Incoming e-mails will all of a sudden get stuck in the Messages Pending
Submission Queue.
The problem does not happen very often, but maybe once every two weeks or
so.
If I try to start the Default SMTP Virtual Server, or the SMTP Service, I
am unsucessful in re-starting.
Could this be a corrupt SMTP engine?
I don't have any related error messages, and I have logging on full.
I have to re-start the server to get my e-mail flowing again.
