Hi Samz,
Thank you for posting in SBS newsgroup.
I wonder how you configure your Exchange Server to route mail, via
Smarthost or DNS Server (usually your ISP DNS Server) only.
When Exchange Server send out email, it will either push all of the
outgoing emails to Smarthost, or query DNS Server for the mx record of the
target domain name and send it. If the route path is not very fast, it will
experience the problem also.
Currently I have the following suggestions for you:
Suggestion 1: Please refer to the KB article to block SMPT open relay.

Suggestion 2: Please rerun CEICW to see how thing goes:
1. Open Server Management, click To Do List, and then click Connect to the
Internet to launch CEICW.
2. On the Wizard, click Next until you see Firewall page.
3. Click to select Enable Firewall, and then click Next.
4. Select all options, and then click Next.
5. Click to select "Allow access to only the following web site services
from the Internet", and then select the websites that you need such as
Windows SharePoint Services intranet site, and then click Next.
6. Click Create a new Web server certificate and input your FQDN as the web
server certificate on Web server name column and then click Next.
7. Follow the instructions to finish the rest steps.

Suggestion 3: Double check network configuration on SBS and client computer:
Please check the DNS Server you set on the SBS 2003 Server, make sure they
all point to your internal DNS Server and set ISP Server in the DNS Server
forwarder lists. Please check the DNS Server on client and make sure it
points to your internal DNS Server.
On SBS server:
External NIC:
IP: assigned by your ISP or your hardware router
Gateway: your ISP or your Hardware router IP
DNS: SBS INTERNAL NIC IP as the only entry
Internal NIC:
IP: Fixed IP
Gateway: None
DNS: SBS INTERNAL NIC IP as the only entry
In the DNS console (dnsmgmt.msc), right click your ServerName and click
properties. In the Forwarders tab, your ISP DNS server IP should be
inputted there.
On workstation inside your SBS local subnet
IP: Assigned by DHCP on SBS
Gateway: SBS internal NIC IP
DNS: SBS INTERNAL NIC IP as the only entry
Suggestion 4: Please try to see if NetBIOS over TCP/IP have been enabled on
the internal server NIC:
1. On the desktop, right-click My Network Places, and then click
Properties.
2. Right-click the appropriate connection and then click Properties.
3. Click Internet Protocol (TCP/IP) and click Properties.
4. Click Advanced, and then click WINS.
5. If you are using a static Internet Protocol (IP) address, click Enable
NetBIOS over TCP/IP. If you have a DHCP-assigned IP address, click Use
NetBIOS setting from the DHCP server.
6. Click OK in the open dialog boxes to close them and to save the new
settings.
Suggestion 5: This also may be an IE issue. I suggest you restore the IE
settings and clear the IE cache on SBS and client computer, clear DNS, IIS
and ISA cache on SBS and check if the issue disappears:
I. Reset IE and clear IE cache:
1. Quit all Internet Explorer windows. Double-click Internet Options in
Control Panel.
2. Click the "Security" tab.
3. In the list box, highlight the "Internet" icon and then click the
"Custom Level" button.
4. In the "Reset custom settings" section, please choose "Medium" in the
drop-down list and then click the "Reset" button. Click Yes to confirm this
operation.
5. Click OK.
6. Click the "Privacy" tab. Click the "Default" button (if the button is
grayed out, let it be).
7. Click the "Advanced" tab.
8. Click the "Restore Defaults" button.
9. Click Apply and click OK.
10. Click the "General" tab, click "Clear History" in the History area, and
then click OK in the dialog box that appears.
11. In the "Temporary Internet Files" area, click "Delete Files" button,
check "Delete all offline content" and then click OK in the dialog box that
appears.
12. In the "Temporary Internet Files" area, click "Settings", and then
click "View Objects" in the dialog box that appears. This will open the
Downloaded Program Files folder.
13. Delete all the object files.
14. Run Internet Explorer again.
II. Clear polluted DNS entry.
1. Secure the DNS cache against pollution
"Secure cache against pollution" is an option under the Advanced tab in
properties of the DNS server in the DNS MMC.
2. Clear the DNS cache
In command line on SBS server: Ipconfig /flushdns
3. Restart the DNS service
III. Clear the cached, compressed copy of the affected files on the SBS
server:
1. Clear the IIS server files:
*. Go to your %windows%\IIS Temporary Compressed Files directory *. Select
all of the content in this directory and delete it.
2. Go to a command prompt, type "iisreset".
IV. Clear the ISA cache following the KB article:

If the problem still persists, please perform a clean boot on SBS as
following:
1. Click Start, click Run, and then in the Open box, type "MSCONFIG"
(without the quotation marks). Click OK.
2. In the System Configuration Utility (MSConfig) window, click to select
the Selective Startup button.
3. Click to clear the check mark from the "Load startup items" below
Selective Startup.
4. Click the Services tab, click to check the "Hide All Microsoft Services"
box, and remove all the check marks from the remained Non-Microsoft
Services. Please note that the Exchange services could be marked as
non-Microsoft. Please do not disable those services.
5. Click OK to close the MSConfig window. Click Yes when you are asked to
restart your computer in order to enable the changes.
6. After restarting, please check whether this issue will reoccur.
