It is entirely possible that the settings on this SBS 2003 server are
correct, and that port blocking by either my ISP [very possible, residential
BellSouth DSL] or my customer's ISP [shared T1] (or both) is at fault. I
just realized that I have not two, but three firewalls in front of my home
office computer: a Westell 2100 DSL modem/router, a 2Wire router, and
Windows OneCare. Though I turned off Windows OneCare, and though I insured
that my two "routers" were passing ports 443, 444, 1723, and 4125 through, I
have yet to telnet to the SBS 2003 server at the customer's location.

If it turns out that one or both of the ISPs are at fault...grrr.

Is there a publicly available computer which I could try to Telnet to, for
testing purposes? Alternatively, would someone be willing to receive an
email with the public IP of the SBS 2003 server in question, so as to test it?

You can telnet to every mailserver on its port 25.
-> telnet mail.xxxxxx.yyy 25

I have run CEICW again [12th or 13th time?], but I haven't gotten better
results.

1) Your DI-524 isn't correctly forwarding to the server -or-
2) There is no server service listening on those ports.

Run the CEICW wizard and fix the DHCP and DNS misconfigurations. Then look
into the DI-524. If you can connect with Telnet locally on these ports, then
the rest is up to the DI-524 and perhaps port blocking by your ISP(s).
