Do you have SBS Standard or Premium (with ISA installed) and two NICs?
This behaviour sounds exactly what you would normally want, and it seems likely you have ISA and it is responsible. I run single-NIC SBSes and block port 25 outbound on the routers from all other IP addresses, to achieve the same result.
Your real problem is probably unconnected with this. Exchange can deliver directly, just not to the smarthost. This sounds like the smarthost requires authentication, or at least to be configured to accept relay requests from your SBS. .
