VPN and RDP to client do-able?

I have a number of clients that spend most of their time remote. When they
are in the office and physically connected to the LAN, we can RDP to the
machines without a problem.
When they go remote, in their home offices, with NAT Routers that forward
ports 1723 and 3389, they can RWW without a problem. They also connect to
the network using a VPN without a problem.
But while connected, we can not connect to them using RDP. it appears all
firewalls are properly set.
Is this approach do-able or do we need a program such as pcAnywhere to
connect to their desktops.
The main purpose of this is for remote support without having them sit there
and click access approvals.
