Thanks for your update!

What is cover page you used? Please try to use the built-in cover pages to
test, please tell me the result.

You can find the built-in cover pages in follows: Server Management console
-> Fax (Local) -> Cover Pages.

Please try to send fax with one of cover pages to test.

And the attachment file with fax mail body in different page is by design
behavior. Since they are different files and will be printed in different
page.
