Thanks for using the SBS newsgroup!
For your description, I understand that you have problem to send fax with
cover page using Outlook 2003. If I am off base, please don't hesitate to
let me know.
Please perform the steps below to try to send a Fax with cover page in
Outlook:
Step 1: Configuring Outlook 2003 to send faxes
===============================
1. Open Outlook.
2. Click Tools -> E-Mail Accounts.
3. Click "View or change existing e-mail accounts", and then click "Next"
4. Click "Add".
5. Click "Additional Server Types", and then click "Next".
6. Click "Fax Mail Transport", and then click "Next".
7. Click "OK" when you receive the following message:
The E-mail Account you have just added will not start until you choose Exit
from the File menu, and then restart Microsoft Outlook.
8. If you want to include a cover page when you send faxes from Outlook,
click "Fax Mail Transport", click "Change", click "Include a cover page
with faxes", and then click "OK".
9. Click "Finish", and then click "Close".
Step 2: Send a Fax
===============================
1. Open Outlook.
2. Click File, point to New, and then click Mail Message.
3. In the To.. field, specify the fax recipient.
4. Type the subject and body of the fax.
5. Click Send.
How about the result?

If the issue persists, please help me collect some screen shots for analyze
when reproduced the issue.
To make a screen shot:
A. Press Alt + Pr Scrn to capture a screen shot.
B. From Start, go to Run, enter pbrush in the Open box, and then click OK.
C. Use Ctrl + V to paste the screen shot to the canvas.
D. From the File menu, go to Save and save as a JPG file.
You can paste the images in the newsgroup or mail it to me at
v-yanniw@xxxxxxxxxxxxx
