I understand that you are worry about losing the existing filter on
Exchange system, if you reinstall IMF on your Exchange 2003.
Analyzing and suggestions:
===============
Generally speaking, you do not need to worry about that as the filter you
referred did not relate with IMF, it is related with Exchange 2003 system,
so it will not be uninstall even if you remove the IMF, the IMF will only
use this filter for IMF working. I test it on my test machine with IMF
installed; the filter will not lose after you install Exchange 2003 SP2.
You only need to reconfigure the SCL setting on IMF on Internet message
deliver option and SMTP virtual Server sections then you can recover all
the setting relate to IMF. All the filters will not be changed during the
IMF reinstall and upgrade to a new version.

In additional, I test on my SBS 2003, it did not ask me to reinstall the
IMF, it might be caused by your IMF version. On SP2 we have many
improvements on anti-spam features; you can refer to the suggestions below
for value adds information:
