Thank you for your reply. Your understanding is really appreciated!

Technically speaking, if this needs to be supported through the firewall,
the firewall should have an SIP filter to parse these SIP traffics through
the firewall and change the content. Therefore, the external client can
know the actual IP it need to connect to and ISA can open that port when
the external client connects. Currently, if you would like to initiate an
Audio conversation, please let the external client to do this. You can send
an instant message to the external client to ask him/her to initiate.

I know that MSN Messenger is a great utility to allow us send instant
message and provide Audio/Video conversations. However, currently we cannot
fully make this specific feature work inside of the firewall. I would
recommend you report this feedback to the development team so they can
consider building an SIP filter to support this special protocol. You can
post your suggestions to
http://register.microsoft.com/mswish/suggestion.asp. Any idea to improve
the product is greatly appreciated and seriously considered.
