No Incoming email, RWW or OWA after Install of ISA 2004 SP1 (SBS S

Following the smallbizserver.net doco on "How to install Service
Pack 1 on SBS 2003", it suggests to install ISA 2004 SP1 prior to
SQL 2000 SP4.
http://www.smallbizserver.net/Default.aspx?PageContentMode=1&tabid=236
Confusing as MS install notes seem to point the other way around.
(And many posts of certain members install sequence seem to follow
SQL 2000 SP4 with ISA 2004 SP1)
First Question:
I guess I am wondering what is "Best Practice" or does it not really
matter what order we do ISA and SQL updates for the Premium part of SBS SP1
Update?
I am doing an install right now of SBS 2003 SP1 for a client. (Been here all
night)
Decided to follow the smallbizserver.net doco as it was written
by "real life experiences" thus trusting it at this stage.
Everything was going fine until I started the ISA 2004 SP1 install.
First issue I hit was one that was recorded in the
smallbizserver.net doco.
"If during the installation of ISA 2004 you get prompted with the
message that ISA 2000 cannot be uninstalled because IISadmin cannot
shut down, DO NOT manually uninstall ISA 2000 as is prompted on the
screen. Change the IISadmin service to disabled and reboot. Restart
the ISA Server 2004 installation again. This will ensure that your
ISA 2000 rulesets will be maintained and reimported back in. "
I did as required and disabled the IISadmin, rebooted. This kills a
few other dependant services on boot up as expected. Then ran ISA
install again. This time I got through the install of ISA 2004 SP1 to the
CEICW, all ISA 2000 stuff seemed to have exported OK, but when I got to Web
Services
Configuration Window of wizard, there was NO Web Site Services in
the box below!! Started to feel like I knew it was going to well to
be true.
OK, I am guessing because IISAdmin service was disabled (thus a few
other services stopped as well) that caused this issue. I tried to enable
then start
IISAdmin service and manually started all other services that had
stopped but were set to Automatically start. No errors, all the
services started. Did not make any difference to CEICW in this
instance (still blank Web Service configuration window), but I decided to
continue to the end of the wizard and see what happened. (I did waste 30 mins
searching for someone else posting this same scenario, but found none)
CEICW completed successful. For interest sake, it said it configured only
the Outlook Web Access, and Remote Web Workplace to have access
through firewall.
What I was pleased about was that by completing the above CEICW, the
ISA 2004 scripts/install continued and then ISA 2004 reported a
successful install. Rebooted
Moment of truth was to run CEICW again, this time all seemed well and I
had all my options returned for Web Services Configuration window.
The wizard again completed successful with the changes I made.
Problem identified: not receiving external internet Email. Can't RWW or OWA
into site. No issues sending out email, Internet access outwards, I can even
RDP into this site which I thought was strange.
I have re-run CEICW many times and each time it completes successful, but
does not change my above problems.
I enabled the additional 3rd party services I needed for client to allow
them to connect to the internet again (Eg Trend Micro AV, Backup Exec
Services, APC UPS services)
I have finished the SBS Premium SQL 2000 SP4 updates for both the default
SQL instance and Sharepoint. I have completed the internal client clean up of
uninstalling old Firewall clients and installing new ISA 2004 FW clients. No
issues here yet.
I can't leave site until I get this working as I can not reliably resolve
this problem connected remotely. Any help/advice appreciated.
Below are some clues from the icwlog.txt Seems to indicate that the ISA
2000 export did not come across that nicely after all?

If it helps, I did a export of ISA 2000 config to a file before I started
the ISA 2004 install process. (And yes AV software, etc was disabled during
this install)
Called MS PSS as well, but on a huge waiting list and I am getting tired.
after been up all night Already sent them ISA Info Report tool from
http://www.isatools.org
Hope this gives everyone plenty of info to narrow down my issue fast
