Thanks for updates.

It seems when sending fax via outlook you need to enter a area code append
with your outlook fax number in TO line.

This issue might be cause in outlook side, you did not set the country
code, you can click the business fax on the outlook, when you add a new
contact, by default you will be prompt to enter a country code and area
code, if you enter the country code, it will be used for all the contacts
you create. You can set this rules in the phone and modem options. From
your description, i am a little confuse, why you not set this rules on
server side, did you also use the modem for dial up connection?

Please note that if you use SBS, this setting should be configured at SBS
side.
