Cannot get IMAP to work

Hello!
I have SBS 2003 SP1 Premium, but no ISA installed yet. Email works fine for
Exchange and OWA, but I cannot get a connection using IMAP. The service is
set to Automatic and it is started.
The server is behind a hardware firewall that forwards port 143 TCP to the
server, I have run the CEICW and added port 143 TCP, and I can Telnet from
the WAN or LAN to port 143 on the server. I tried this with Outlook Express
on my workstation and on the server itself. Neither one works.
I get this error when I try to connect (I added the xxx to mask my domain in
this post):

Server: mail.hillxxx.net

Protocol: IMAP
Port: 143
Secure(SSL): 0
Code: 800cccd1
There are no errors in Event Viewer.
I have tried several suggestions found while Googling, including using
"ntDomain\ntUserName\ExchangeAlias" as the account name (with my info in
there, of course!). If I use any variation of that suggestion
(hillxxx/gregghill/gregghill, hillxxx/gregg hill/gregg hill, hillxxx/gregg
hill/gregghill, or hillxxx/gregghill/gregg hill), I get the same error:
Logon failure: unknown user name or bad password.

The answer is hiding from me!
Any ideas on why IMAP fails?
