I had disabled all the antivirus services manually in previous attempt.
Only a few items may have been running, such as MGE UPS service but
I didn't think that would interfere. Would it? The servicepack install
didn't seem to be affected.

Looking at the document you suggested, after clicking on "finish" the
installer fails almost immediately. There is no process when it says
it is uninstalling ISA2000. The document refers to a number of steps
to preserve cert, but there isn't much how ISA2004 might be affected.
It appears to be "fill in the wizard then click finish". Unfortunately, it
doesn't appear to work in my case.

To recap, SBSsp1 was installed first, followed by WindowsUpdate.
There was a time gap between SBSsp1 install and the Premium install
esp. SQL and ISA2004 due to the time for the Premium CD to arrive.
Does this cause a problem?

In the Event Log I can't seem to find any item which could provide
some clue as to why ISA2004 couldn't install/upgrade. ISA2000
is installed with the latest service pack.

Now that I've installed SQL update, followed by SBSsp1 (installed
the servicepack again), has this damaged this SBS2k3?
