Can you create a proxy which acts on the virtual host? This will then
work as follows:

Remote client connecting to public ip --> router --> port mapping to
internal webserver --> requested host matches rule --> proxy requests
to SBS internal server.

Good instructions on doing this under Apache are found at:
http://3cx.org/item/38 - the issue here is that you need to ensure the
SSL is set up correctly.
