Messenger 7 Video and Voice on SBS2003 premium

Hi
We use messenger 7.0 to communicate around our office and to other workers
at remote sites. We recently installed some web cams to try video and audio
calls on messenger.
Our remote users VPN into our server to access exchange, file shares and
local applications like MS Project Server. We have UPnP activated on the
routers on either end of the VPNs and they are on the latest firmware. We
are also on BT 2mb Business Broadband.
Advanced connection information in messenger shows that while on VPN the
UPnP setting of the router is not recognised by messenger.
What we see is
- PC1 logged on to LAN inside ISA2000 can initiate a video and audio call to
PC2 which is connected outside the LAN via VPN. Video and sound are
tranmitted out from PC1 to PC2. Video only is transmitted back from PC2 to
the local LAN.
- If we connect PC1 and PC2 outside the SBS network over the open internet,
video and voice works no problem. So it's not how the router at the remote
addess is set up. It my simple mind it has something to do with ISA2000.
Questions
- why does video and audio only work one way?
- is there something in ISA that I need to do to get audio working both
ways?
- will SBS 2003 SP1 (ie ISA2004) fix the mismatch between using messenger on
SBS and using it on the open internet?

PS yes I know ENat exists (http://www.easyfp.com/msn-messenger-directtalk/)
but I am wary about adding anything else to our network that opens ports or
tunnels outside the control of ISA.
PPS I have heard that since messenger 7 uses SIPS, ISA2004 needs to have a
SIPS gateway to make this work
